Dennis Evans, 59, of Goshen, died Dec. 28.
Evans was an assembly line worker for Evercoat.
He is survived by his wife Connie Farnsley, children Michelle and John Evans, mother Virginia McKnight and brothers Donovan and Roger Evans.
Services 10 a.m. Jan. 4 at Evans Funeral Home in Milford. Interment at Mt. Pisgah Methodist Church Cemetery in Dry Ridge, Ky.
