Jazmine Sara Smith, 29, Columbus and formerly of Milford, died Wednesday, Jan. 13, 2010 at St. Ann Hospital in Westerville. She was born July 3, 1980 the daughter of Patricia Smith. She was raised by her grandparents, Dan and Delores Kaaz.
She was a graduate of Milford High School and was a certified day care assistant at Nikou Learning Center in Columbus.
She is survived by her grandparents; her mother and step-father, Billy and Patricia Stallworth; one brother, Jesse Aaron Smith; one sister, Cynthia Grace Smith; two step-sisters, Candice Marie and Tiffany Nicole Mobley and several cousins.
Graveside services were held Jan. 19 at Graceland Memorial Gardens Chapel in Milford.
